Supporting Hospice Patients with More Intensive Needs
Hospice care is the leading choice for comfort care at home for people with advanced illness. It offers comfort, support, and dignity when physicians feel the patient’s life expectancy is limited to six months or less if the illness follows its normal course. Our Partners specialize in improving the quality of life and preventing and relieving suffering for these patients, as well as offering them and their families support and information regarding their care.
We recognize that every person, every illness, and every family is unique, so we create individualized care plans based on the wishes of each patient and on our foundational belief in the right of every person to manage an advanced illness in their own way.
Your Needs Met in Our Care
If your loved ones’ needs intensify, the hospice team may recommend transferring him or her to the inpatient level of care. The goal of inpatient hospice care is to control severe pain and symptoms so that your loved one can return home to their family and familiar surroundings, if possible, and resume hospice care at home.
